Because it is designed so that the border of the lips, which is the darkest color, is the center of the cursor.
Just trace the line of the mouth of the line drawing to easily get a beautiful shape.
There is also a curve tool that allows you to draw beautiful curves.
If you set the shape of the brush-type cursor to "brush size and dot" in the environment settings, you can see the center of the cursor and it becomes easier to use.
Since it is assumed that the line is drawn from left to right (right-handed)
If the opposite is easier to use, flip vertical the brush tip.

環境設定でブラシ系カーソルの形状を「ブラシサイズと点」などにするとカーソルの中心が見えて使いやすくなります。
左から右に向かって線を引く（右利き）想定をしているので
逆の方が使いやすい場合はブラシ先端を上下反転してください。
