Here is an example of an empty Template. It's fairly simple, as I like things simple, but not so simple that I won't be able to know what is going on if I forget the words.
Here is a format that has been used a bit. I like to colour code and label my layers so that I won't loose them. I haven't put that into the Template so their people can use the layers to their liking.
Also please remember this is just how I do my Storyboards, you may find a way that will work better for you! :D
